Step 1 -
Start by creating a new document, 400x50 pixels; filling it white.

Grab the rounded rectangle shape and drag out something similar to that in the diagram.

Rasterize this layer, by right clicking on the layer, and selecting rasterize. Then with the rectangular marquee tool, select an area just above the bottom corners.

You should now have something like ours.
